To better serve you, please read the checklist below and bring the necessary documentation to your first appointment:

1. Complete and sign all downloadable forms before your appointment and be sure to bring them with you. DOWNLOAD FORMS

2. Informed Consent and Arbitration Forms will be given to you to sign at the time of your appointment.

3. Full payments in the form of check, cash, Visa or Mastercard are expected at the time service is rendered.

4. Please note that I do not bill to insurance companies and do not accept insurance, however I will gladly provide a statement that you can submit to your insurance company should you wish to try for a reimbursement.

5. Please allow approximately one and a half hours for your initial visit and one hour for subsequent visits.

6. Wear loose and comfortable clothing to your appointment.
